Before your first visit, please print and fill out the following patient forms and bring them with you:
- Consent for Treatment
- Adult Health Information Form or Child Health Information Form
- Insurance/Patient Info Form
- Notice of Privacy Practices – Acknowledgment
- Medical Privacy Form – this is for your records; you do not need to bring it in
- Primavita Fees and Payment Policies Acknowledgment Form
Your first visit will take anywhere from 1.5 to 2 hours to give Dr. Shinsato an opportunity to learn your medical history, understand your current concern, and perform a physical exam. We may order laboratory and diagnostic tests, if necessary.

Primavita Family Medicine accepts cash, personal checks, health savings account cards and major credit cards except American Express.
If you have health insurance, please call our billing company, Puget Sound Medical Billing at 206-434-6317 so we can determine your benefits. You could also email them at psmb@comcast.net. We are currently an in-network provider with Premera and Regence.
We will be happy to fill out your health insurance form so you can submit it to your insurance company for reimbursement for out-of-network benefits. Please note that payment is required at the time of your visit. Worksheet to determine your out of network benefits
For patients without health insurance or with a high deductible insurance, we offer a convenient 50-50 Health Plan, and other cash options, which provides you with cost-effective healthcare. Call us at 425-273-0741 for more information.
